Step 1: BenaVest On-Boarding Forms
Complete the On-Boarding Forms to set up your agent profile within Benavest. These forms can be completed electronically through the below box and include an Agent Data Form, a Carrier Selection Page, a Commission Set-Up Form, and a W9 Form.
This information will help our Contracting Department complete a profile for you, help us send you the correct carrier contracting, and create your Personalized Marketing Platform.
